1. About Folio
Folio is a patient-controlled longitudinal health record and AI-assisted organization service currently operated by Michael Dulin (“Folio,” “we,” or “us”). These Terms apply to foliohealth.app, the Folio early-access service, and Folio’s patient-authorized health-record connection features.
2. Eligibility and early access
You must be at least 18 years old and able to agree to these Terms. Early-access and sandbox features may change, be interrupted, or be withdrawn while we test safety, usability, and interoperability. We may limit enrollment or require an invitation.
3. Your account and authorizations
- You are responsible for the accuracy of your account information and for protecting access to your account and devices.
- When you connect a health system, you authorize that organization to provide your records to Folio through its own login and SMART on FHIR authorization process.
- Folio requests read-only clinical access. You may revoke future access through the connected health system. Revocation does not automatically erase a copy you previously imported into Folio.
4. Your health information
You retain rights in the health information you bring to Folio. You direct which systems to connect, which retrieved records to accept into your record, and what to export or share. You grant Folio permission to host and process your information only as needed to provide, secure, support, and improve the service for you, subject to our Privacy Notice.
5. AI-assisted features
Folio may use artificial intelligence to organize records, identify possible relationships, summarize source material, and draft questions or communications. AI output can be incomplete or wrong. It is labeled as AI-assisted, should be checked against the source record, and must not be used as the sole basis for diagnosis, treatment, medication changes, or urgent decisions. A clinician should make clinical decisions with you.
6. Not for emergencies
Do not use Folio for emergencies or time-sensitive clinical communication. Call 911 in the United States, contact local emergency services, or use the communication method your healthcare organization directs you to use.
7. Sharing and communications
Nothing is shared with a clinician, caregiver, or other person unless you take an explicit sharing action. Drafts prepared by Folio are not sent automatically. You are responsible for reviewing the content, recipient, scope, and expiration of anything you choose to share.
8. Acceptable use
You may not use Folio to access another person’s information without lawful authorization, interfere with the service, introduce malicious code, evade security controls, misrepresent AI output as clinician-authored, or use the service in violation of law or another person’s rights.
9. Export, deletion, and closure
You may request a copy of your Folio data or deletion of your account data by contacting us. Revoking a health-system connection stops future access but does not itself delete imported records. Deletion and limited retention are described in the Privacy Notice.
10. Availability and disclaimers
Early-access software is provided on an “as is” and “as available” basis. Health systems control their own portals, data, authorization, and availability. To the extent permitted by law, Folio disclaims implied warranties and is not responsible for clinical decisions, source-record errors, third-party outages, or actions you take based on unverified AI output.
11. Changes
We may update these Terms as the service develops. We will post the effective date and provide additional notice when a material change requires it. Continued use after an update means you accept the revised Terms.
